Application for change of Name or Surname

Eligibility of applications: in the Italian legal system, the change of name or the modification of the surname are of an exceptional nature, therefore requests may be accepted only and exclusively in the presence of objectively relevant situations, supported by adequate and substantial documents and solid and significant reasons: surnames or names RIDICULOUS, SHAMEFUL or THAT REVEAL NATURAL ORIGIN.

In no case may the attribution of surnames of historical importance or in any case such as to mislead about the applicant’s belonging to illustrious or particularly well-known families in the place where the birth certificate or the place of residence of the applicant are registered.

Surname of the woman: the married Italian WOMAN keeps her maiden name in her passport and in other documents concerning her by law. You can (but it is not mandatory) request that the married surname be added to your maiden name with an annotation on page 4 of the passport.

Application procedure:

Request for change of name or surname for adults:

The compatriot must go to the consulate of the Singapore Embassy by appointment booked online via PRENOT@MI.

Compulsory documents to be presented:

  1. Request from the interested party on a special form that can be downloaded from the website of the Prefecture in charge (the province to which the AIRE Municipality is affiliated);
  2. Self-Declaration available on the website of the Prefecture in charge;
  3. Photocopy of the passport;
  4. Extract of the birth certificate requested from the Municipality;
  5. Letter of consent by who could be against the surname change (i.e. spouse);
  6. Photocopy of the spouse’s passport;
  7. Payment of the stamp duty for sending the application.

 

Application to change name or surname for minors:

Parents of the minor must go to the consulate of the Singapore Embassy by appointment, which can be booked through the Book Online service.

Compulsory documents to be presented:

  1. Application by the concerned applicant on the appropriate form (change of name / surname of minors) downloadable from the website of the Prefecture in charge (the province to which the AIRE Municipality is affiliated);
  2. Self-Declarations available on the website of the Prefecture in charge (completed by both parents);
  3. Photocopy of the minor’s passport;
  4. Extract of the minor’s birth certificate requested from the Municipality;
  5. Photocopy of the parents’ passports;
  6. Payment of the revenue stamp for presenting the application.

The consular office will forward the application to the Prefecture in charge which will decide whether or not to accept it. Following a positive opinion, the Prefecture will send a notice containing the summary of the application (provisional decree) which must be posted on the consular notice board for 30 days upon payment of a stamp duty by the interested party.

At the end of the aforementioned period, the consular office will communicate the posting to the Prefecture, which will issue the definitive decree to change the name or surname with the request for the payment of an additional stamp duty through the consular office. This decree will subsequently be sent to the municipality of birth of the interested party with a request for registering the change of name or surname. Once the procedure is finalized, it will be possible to accept the interested party’s request for the issue of new documents with new details such as the passport or tax code.
